Please complete this application if this is the first time you are requesting disability accommodations at Rocky Vista University.

Upon submission of this application, you must schedule an Intake Meeting via Navigate with Disability Support Services.

It is not required to submit disability verification documentation at this time; however, documentation will be required prior to completing the eligibility determination process and being approved for accommodations at RVU.
